Stop the Job - Accountability/ Leadership
Liam Elliot - Large amounts of wood with nails protruding were left in the walkway in front of the PVC at the laydown area, creating a tripping hazard. All the wood was gathered and taken to the appropriate skip bin.

Gemma Hook-Basher - Raised a incorrect call up point in a morning meeting with the client

Thomas Hill - 350 control regulator wasn’t regulating properly, potentially for compressor failure - Notified supervisor and passed on for fitter to inspect and repair.

Tim Alsop - After completing collar on hole, and about to start drilling, geologist arrived and parked directly below/ behind sump putting themselves and vehicle in the line of fire of potential projectiles from outside return.

Lachlan Kahl - Always compliant with impact resistant gloves when doing any manual handling gloves.

Marcus Brown - Whilst conducting a worksite inspection I found noncompliant grinding discs in the toolbox. Had the potential to fail and projectile could hit someone's body or hurt their hands. Took the discs and disposed of them before explaining the requirements with prohibited tools to the crew.

Connor McCafferty - Bit pot handle was damaged, resulting in an exposed edge, and no suitable place to put hands when carried, chance of a serious hand / finger injury. Informed fitters and got it re- welded and grinder back to suitable use
