Value Award (Accountability/ Leadership)
Nicholas Thurston - Sample splitter is heavy and presents a manual handling hazard when loading it for transport. Discussed at client PSI meeting and presented as a "Safety Activity Idea Generation" on 26.12.25. Strong consideration to reduce bulk and weight by having a single tier 50/50 sample splitter for the rig.

Marcus Brown - Found excessive wear on sample hose whilst conducting a site inspection during maintenance task with Supervisor. Nearly due for change so after liaising brought change out forward.

Robert Gatti - Emergency stop button was not working correctly, creating the risk it would not activate in the case of an emergency.

Michael Kelly - Faulty coolant temp sensor. Displaying ambient temp, not coolant temp. Risk of equipment damage.

Stop the Job - Accountability/ Leadership
Brett Evans - Incorrect gloves being worn while using torque wrench potential for hand injury. Job stopped , Field leadership held around glove selection and use of torque wrench. Correct gloves worn to continue task

HandSafe Winner (Accountability/ Leadership)
Samara Church - Battery left in power tool in toolbox for shift change. Potential Hand Injury lying in wait. Immediately responded by removing battery form power tool, placed tool and battery in correct homes.
