Zane Candian - MKY20361 trailer plug and MKY546 trailer both have faulty wiring and temporary tape fixings. Trailer lights not working which could cause an incident to occur
Christopher Goodwin - Whilst conducting an inspection on the working at heights gear, I noticed the guide on the retractable lanyard had broken off and the lanyard did not retract back onto the reel correctly. Tagged out of service, informed driller to order new one, also informed supervisor
Thomas Davies - No water jugs on crew vehicle. Used bottled water whilst waiting for crew member to bring new jug and additional jug ordered. Very important for this time of year with the monthly FRE being hydration.
Charlotte Buchanan - No secondary retention on cupboard doors. Had rattled loose when driving down corrugated roads. Potential for items in cupboard to fall out. Loose items on deck can then lead to items falling off truck and can eventually lead to a vehicle interaction. Submitted to have consideration to have secondary lock or walkways that stop items falling to road.
Joseph Chahoud - Mercury tilt switch was not set to 15 degrees on KL. During prestart noticed that KL claw is still operating past 15 degrees which might cause a dropped rod. Told Driller and entered work order for issue to be rectified.
Stop the Job - Accountability/ Leadership
Tom Black - Discovered one of the crew had bypassed the deadman switch on the rod handler. Stopped the job immediately. Removed the tape and called the crew in to have a robust discussion about bypassing safety functions. They will not be doing that again.
Ryan Kennedy - Observed Jack legs down near ground whist tramming no 360 conducted before tramming. Potential equipment damage. Stopped the job and rectified immediately

Anthony Spalding - Frays on wireline splice to overshot. Potential to cause hand injury. Isolated and repaired wireline
Near Miss
Ray Arnel - While travelling, the IP experienced a flat tyre. It was identified that the IP did not have the required amount of water for travel and lacked sufficient communication capability while conducting a water run.
Immediate/Temporary Controls: Supervisor discussed with crew when completing a fuel run to make sure, they have enough water and a sat phone available when they have to no service
