Brett Evans - While conducting PTO on sample hose repair noticed wire sticking out of sample hose from where it had been cut. Potential for wire to stab hand or finger resulting in injury. Stopped job, informed crew to cut wire, spoke about previous incident involving wire stabbing someone’s hand.
Phil Pratt - Offsider forgot to put impact gloves on before grabbing over shot to guide down hole and remove dry release. Stopped the job and explained to offsider why we need to wear gloves when handling overshot due to the amount of pinch points. Performed a take control with offsider and completed the job safely.
